I even tried removing Office as suggested in another forum and then reinstalling it. Still the same problem. I'm running El Capitan on a MacBook Pro. I have the same problem. In your login keychain file under Library, Microsoft places a long alpanumeric file called Microsoft Identity when you install Office 2016 for Mac. Microsoft told me to delete this key and restart my Mac. Make sure your apps are closed. Two new keys were created, Microsoft Identity 2 and Microsoft Cache 2. I reopened my Excel and then a recent document and the keychain popup was gone. Because you have a Microsoft account and a perpetual key for Office for Mac, Microsoft wants you to verify your identity. But once the original key is deleted, the issue is gone.
